You're using Outlook.  I assume you probably have a filter rule set up
for the list.  Check to make sure you don't have it set to continue
filtering after the rule is applied -- this could cause one copy to show
up in your Inbox and one in the appropriate folder, or even multiple
copies in the same folder if it matches multiple rules.
